The terms that apply when you use Dog Import Form and order our paid CDC Dog Import Form filing service from Visamundi SAS.

These terms apply to the website Dog Import Form and to the paid filing service sold on it. By placing an order, you accept them. Please read them before you pay.

In these terms, “we” and “us” mean Visamundi SAS. “You” means the person who places the order.

2. Independence

Dog Import Form is an independent private service. We are not the US Centers for Disease Control and Prevention (CDC) and we are not affiliated with, or endorsed by, the CDC or any other US government agency. The official CDC Dog Import Form is free on cdc.gov (opens in a new tab), and you can fill it in yourself without using our service. Our fee pays for our own work only; no part of it is a government fee.

3. The service

For each dog in your order, the service includes:

  • a check of your dog’s eligibility and CDC pathway, based on your answers;
  • a review, by a member of our team, of your answers and of the documents your pathway requires;
  • filing of the CDC Dog Import Form on cdc.gov on your behalf, as the CDC allows a third party to do;
  • delivery of the CDC receipt to your file page and an email when it is ready;
  • a new filing if your flight date or arrival airport changes before you travel;
  • support by email at contact@dogimportform.com.

The service does not include booking an animal care facility, a vet appointment, a titer test or a flight, nor obtaining any vet certificate or government endorsement.

We start as soon as your file is complete, meaning paid and with every required document accepted. We keep you informed by email. We do not commit to a fixed processing time.

4. Price and payment

Our service fee depends on your dog’s CDC pathway. It is shown before you pay and is calculated by our server from your answers. Fees are in US dollars. Your card may display the amount in your local currency; your bank or card issuer may apply its own conversion rate or fees.

Payment is by card only, in US dollars, through a secure card form provided by Stripe. We do not see or store your full card details. Your order is confirmed when payment succeeds, and you receive an email with your file reference.

5. Your obligations

You agree to:

  • give accurate, complete and up-to-date information about yourself, your trip and your dog;
  • upload genuine, readable documents that belong to your dog;
  • tell us as soon as possible about any change to your trip, especially the arrival date or airport;
  • check the receipt we deliver and tell us at once if anything looks wrong.

You remain responsible for meeting every requirement of the CDC, USDA, US Customs and Border Protection, your airline and any state (such as Hawaii) that applies to your dog: vaccinations, certificates, titer tests, animal care facility bookings, arrival airport and the dog’s health on the day. Our check reduces the risk of mistakes; it does not replace those requirements.

6. Withdrawal right and refunds

Full refund if we cannot file. If we cannot file the form, including when our check shows that your dog cannot enter the United States, we refund the full fee.

14-day withdrawal right. Under French consumer law, you may withdraw from your order within 14 days of payment, without giving a reason, as long as we have not filed the form. To withdraw, email contact@dogimportform.com with your file reference, or use any other clear statement. We refund you within 14 days, using the same payment method.

When you pay, you ask us to start right away. By paying, you expressly ask us to begin the service before the 14-day period ends, and you acknowledge that you lose your withdrawal right once we have filed the form, as the service is then fully performed (Article L221-28 of the French Consumer Code).

After filing, the fee is not refundable, except when the problem comes from our own error or where the law requires a refund. A change of flight date or airport before travel is handled by a new filing at no cost, not by a refund.

7. Liability

We perform the service with care and according to the CDC rules published at the time of filing. We are not responsible for:

  • decisions taken by your airline, US Customs and Border Protection, the CDC or any other authority;
  • consequences of inaccurate or incomplete information or documents you provide;
  • changes in official rules after we filed your form;
  • delays or failures of the CDC’s online system or of third-party services outside our control.

To the extent permitted by law, our total liability for an order is limited to the fee you paid for it. This limit does not apply in case of gross negligence or willful misconduct on our part, to personal injury, or where the law does not allow it. Nothing in these terms limits the rights you have as a consumer under the law.

The pages on this site give general information, checked against official sources. They are not legal or veterinary advice. See our disclaimer for details.

9. Personal data

We process your personal data to provide the service, as described in our privacy policy, which is linked in the footer of every page.

10. Complaints and mediation

If you have a complaint, please email contact@dogimportform.com first, with your file reference. We will do our best to find a solution.

If we cannot resolve it, you may refer the matter free of charge to a consumer mediator, as provided by the French Consumer Code. [Consumer mediator: to be completed by Visamundi SAS before launch]

11. Governing law and courts

These terms are governed by French law. In case of a dispute, and if no amicable solution is found, the competent courts are those determined by the applicable consumer protection rules. If you are a consumer living in another country, you also keep the protection of the mandatory rules of the country where you live.

12. Changes to these terms

We may update these terms, for example when the service or the law changes. We publish the current version on this page. The terms that apply to your order are the ones in force on the day you paid.
